Fear of surgery in fissure.

Dear Sir, My husband aged 34 years has problem of fissure .Alophaty treatment of the same is being continued from last 20-25 days.Pain almost got cured from pain killers advised by doctor but once pain killers were stopped again pain has started.So doctor advised to continue pain killers for next 10 days.In addition to this he is using ointment & gels advised by doctor. How much time it takes to cure fissure?Would the pain again start after discontinuing painkillers.Any further problem would evolve from this.Would this all lead to surgery?He is a consultant so has continouis sitting job.His food habits are not bad.He is problem of constipation since long time.Kindly advise.
