High cretanine diabetic and suffering from hypertension

My mother aged 65 years suffering from diabetic from last 6 years and suffering from hypertension since 1989. Recently this month she has been diagonised with high cretanine of 1.70 mgt. Please suggest and also suggest the diet plan
